Zaragoza Elementary School: Follow Up Procedures

Fall
Pattern A:
• Look through follow up list. Who was having regular follow ups? Are
they registered in school/ or did they move away? If not in school take off
follow up list.
• If they are back in school, check to see how they are doing (especially
if they have asthma-do they have meds?)
• Purge files. (Those that have graduated from 6th grade. If they are
not out of elementary do not purge the file, they may come back to the school.)
• Toward the end of your clinical, you may start to identify some new
people that may be having excessive absences.
• Come up with plan of care for children receiving follow up visits.
• Follow up on children referred from Agape.
• Keep logs/files updated.
• Communication with Mr. Kemp, Agape, Amy important.
Pattern B /Spring
Pattern A:
• Look at follow up list to identify/familiarize with those children
we are currently doing follow-ups on.
• Compare new absence list with old. (Amy can print a new list.)
• Identify any new students from the excessive absences list.
• Are these children already “in the system?” (i.e. have
we done follow ups in the past-do they have a chart?)
• Come up with a plan of care for children receiving follow up visits.
• If this is going to be an on going follow up, let the family know.
• Follow up on children referred by Agape.
• Keep logs/files updated.
• Communication with Mr. Kemp, Agape, Amy important.
Spring Pattern
B:
• Do all of above
• For families receiving ongoing follow-ups let them know these will
be D/C over summer. They can still get medical treatment/meds from the Agape
Clinic through the summer.
